By using this Website and/or reading or commenting on this Website’s content, e-mails, social media, videos, and other products accessed or available through this Website, you are voluntarily agreeing to these Terms & Conditions, and you are legally agreeing that you have read, understand, and fully consent to the terms below. Certain features of the Website may be subject to additional guidelines, policies, terms, or rules, which will be posted on the Website in connection with such features. All such additional terms, guidelines, and rules—including, but not limited to, the Privacy Policy and Disclaimer—are incorporated by reference into these Terms & Conditions.

You’ve been told your labs are normal. You’ve been told it’s just stress. You’ve been told it’s just aging. You’ve been handed a prescription you didn’t ask for, a referral that went nowhere, or a shoulder shrug that cost you a $40 copay.

Somewhere along the way, you started to wonder if you were imagining it.

1. Practice Scope.
Wellness Continuum is a telehealth-only functional medicine practice serving adults 18 and older. Medical services are consultative and are not a substitute for emergency care, urgent care, hospital care, or routine primary care. All patients must maintain an established primary care provider (PCP) throughout their care with Wellness Continuum. Wellness Continuum does not provide primary care, emergency or urgent care, or ongoing dosage adjustments for chronic medications as part of primary care medication management.

2. Medical and Non-Medical Services.
Wellness Continuum offers medical telehealth services, educational content, digital products, memberships, supplements, and wellness support services. Some wellness support services may be provided by independent contractors, such as a health coach or nutrition professional. A patient-provider relationship is established only when a licensed practitioner has reviewed your information, agreed to see you, and you have completed the required intake and consent process. Visiting the website, downloading materials, or sending a message through the Site does not, by itself, create a patient-provider relationship.

3. Telehealth Requirements.
Maryland telehealth rules require identity verification, informed consent, secure and private technology, and safety procedures for emergencies that arise during telehealth encounters. For synchronous visits, you may be asked to confirm your identity, physical location, a callback number, and who is present with you during the session. Telehealth is not appropriate for every issue. Wellness Continuum may determine that an in-person evaluation, emergency care, urgent care, specialty care, or care from your PCP is better suited to your needs.

4. Emergencies and Urgent Concerns.
Do not use the website, email, portal, text, or telehealth services for emergencies or urgent medical concerns. If you are experiencing an emergency, call 911, go to the nearest emergency department, or seek immediate care locally.

5. Text, SMS, and Electronic Communications.
Wellness Continuum may communicate with you by email, portal message, phone, or text/SMS for scheduling, reminders, administrative updates, and other service-related communications. Some communications may be sent through secure systems, while standard SMS may not be encrypted and may carry privacy risk. By providing your contact information and opting to communicate electronically, you agree to receive these communications, subject to your preferences and applicable law.

6. Fees and Payment.
Wellness Continuum is a cash-pay practice and does not bill insurance. You are responsible for payment of fees for visits, programs, memberships, digital products, and other services you choose to purchase. Payment plans for services are available. Specific pricing, refund and cancellation terms, and any package details will be provided at or before purchase.

Medicare & Medicare Advantage Beneficiaries. At this time, Wellness Continuum does not see Medicare or Medicare Advantage beneficiaries. This will change at a later date, and this Section will be updated when that change takes effect.

7. Intellectual Property Rights.
Our Website, programs, products, services, and all materials available on or through it are solely owned by Practice. This material includes, but is not limited to, the design, layout, look, appearance, and graphics. You may not copy, reproduce, modify, republish, upload, post, transmit, or distribute any documents or information from this Website in any form or by any means without express written permission from Practice. Any unauthorized use of the materials appearing on this Website may violate copyright, trademark, and other applicable laws.

8. Independent Contractors.
Some services associated with Wellness Continuum may be provided by independent contractors rather than employees, such as health coaches or nutrition professionals. Independent contractors are individually responsible for the licenses, insurance, consents, notices, and legal or professional compliance obligations that apply to the services they provide.

9. Limitation of Liability.
To the maximum extent permitted by applicable law, Wellness Continuum is not liable for indirect, incidental, special, or consequential damages arising from the use of the Site or reliance on general educational content. Practice assumes no responsibility for the timeliness, deletion, misdelivery, or failure to store any user communications or personalization settings.

10. Jurisdiction and Governing Law.
This Website is controlled by Practice from its offices within the State of Maryland. By accessing this Website, Practice and you both agree that the statutes and laws of the State of Maryland, without regard to the conflict of laws principles thereof, will apply to all matters relating to the use of this Website. Practice and you both agree and submit to the exclusive personal jurisdiction and venue of the state and federal courts sitting in the United States District Court for the District of Howard County, Maryland.
